Prior to completing the form, ensure you have the following documents:

A copy of your Drivers License (required for background study)
MMR – proof of vaccination or immunity – required
Varicella – proof of vaccination or immunity- required
TB – 2 step Mantoux, T-spot or QuantiFERON Gold – required
Hepatitis B – proof of vaccination or immunity (optional but preferred)
COVID-19 – proof of vaccination (optional but preferred)
Tdap – proof of vaccination within 10 years (optional but preferred)
Influenza -proof of vaccination for the Flu Season – September-April (optional but encouraged) if you choose not to be vaccinated you will be required to wear a mask within 3 feet of a patient or patient care area.

Attitude

• Introduce yourself and call people by name.
• Recognize that patients are the reason we are here. Be accountable to them.
• Project support of the hospital; its mission, vision, values, policies & procedures.
• Report to work as scheduled prepared to carry out your responsibilities and displaying a positive attitude.

Appearance

• Personal appearance is professional and appropriate for position.
• Wear identification at all times.
• Maintain a clean and comfortable environment.

Managing Up

• Manage up coworkers, leaders, and yourself.

• Advocate that patient rights are protected in respect to their emotional, spiritual, and physical needs.
• Never say anything about a patient that you would not say to the patient.
• Use AIDET for every interaction with every customer.

Commitment to Coworkers

• Treat others with courtesy and respect.
• Welcome new staff. Set an example of cooperation within the workplace.
• Address problems or misunderstandings with the appropriate person or supervisor/manager.
• Discuss conflicts with coworkers directly and privately.

Of course, the Standards of Behavior cannot address every situation, and issues will continue to evolve as our organization grows.

Confidentiality Statement

A health record is the property of the health facility and is maintained to serve the patient, the health care provider and the institution in accordance with legal, accrediting and regulatory agency requirements.

The information contained in the health record belongs to the patient, and the patient is entitled to the protected right of information. All patient care information shall be regarded as confidential and available only to authorized users. In addition, all information in regard to patient finances and billing information is considered confidential.

Through the course of working in and being exposed to the health care environment, you may hear and/or have access to confidential information regarding patients at our hospital. This information, specifically diagnostic data, financial data and other information, even the fact that a patient is in the facility, cannot be discussed outside the facility, nor even within the facility with coworkers or others who have no legitimate, work-related reason for knowing it.

All requests for release of medical information regarding patients presently in the facility should be referred to the appropriate nursing personnel. Personal inquiries regarding a patient should be referred to the family. All other requests for release of information should be forwarded to the Health Information Management Department. No employee, volunteer, student, or other health worker may release patient information unless management has specifically authorized them to do so.

All individuals engaged in the collection, handling or dissemination of patient health information are responsible for protecting the confidentiality of this data. Shred paper when disposing of any information which identifies patient/employee names or other identifying data.

Proven violation of confidentiality of patient information may be cause for immediate termination of access to further data, and immediate termination of any employer-employee, volunteer, or other relation ship with the facility with prejudge against rehire. In addition, improper release of information, either orally (including telephone inquiries) or written can result in legal action against the individual and/or hospital.

Department-specific confidentiality policies or procedures will be discussed within the department.

Additionally, patients have a right to privacy, such as having employees, health workers and other visitors knock and receive permission to enter their room, keeping their door closed, curtains pulled, and body covered during cares.

Written, computer or verbal information will be kept out of public view or hearing and shall not be left unattended where unauthorized employees and/or the public would have access. Confidential information will not be left on answering machines.

Personnel Information regarding facility employees is also confidential and is not to be released by unauthorized individuals. This includes such information as an employee’s home address and telephone number. Requests for information regarding employees should be forwarded to the Human Resources Department.
